2025-26 Our Minds Matter Fall Impact Poll

Introduction & Informed Assent

Hello! We’re inviting you to fill out this brief survey because you are part of your school’s Our Minds Matter club. We want to learn more about how well this program works, and that means we need to ask you, the experts, about how your thoughts, attitudes, and behaviors may have changed over time. You’re asked to take a brief survey now and again in April (toward the end of school year). We want to learn how your thoughts, attitudes, and behaviors may be different at the end of the program. Both surveys will take around 5-10 minutes.


Both surveys are anonymous and confidential, which means your name does not go on the surveys and we will never be able to link your answers to you. We will not know which answers are yours, and your answers will be combined with those from other young people. Both surveys are voluntary and you may skip any question you do not want to answer. The information you tell us will be used to help make this program better in the future.
